Introduction

Cherokee Nation Home Health Outreach is a department of Cherokee Nation Home Health Services (CNHHS).

Cherokee Nation Home Health Outreach obtained approval through the state of Oklahoma and admitted its first patient in 1996. We have continued providing a vital service to our consumers since inception.

We provide a broad range of home and community health services to persons of all ages.

Our services are covered by two separate state plans:

  • Oklahoma State Personal Care Plan, and
  • the Advantage Waiver Program.

General Eligibility

Home Health Services OklahomaAll persons who are currently on Medicaid and meet Department of Human Services (DHS) Medicaid Guidelines could be eligible for these programs. If you are currently on Medicaid, contact your local DHS office to access your eligibility. If you are a new client and need to access these services, call the Long Term Care Authority at 1-800-435-4711.

Below, we will describe each program, the individual eligibility requirements and the services offered under each program. All persons approved for the State Plan Personal Care Program and the Advantage Waiver Program must meet DHS guidelines and be approved by DHS before services can be performed.

State Plan Personal Care

The State Plan Personal Care Program is a Medicaid funded program that provides personal care in the home of any person, regardless of age, who meets Medicaid financial and medical eligibility guidelines as determined by the Department of Human Services.

PROGRAM ELIGIBILITY GUIDELINES

  • Must meet medical criteria.
  • Need assistance in meeting daily living needs.
  • Individual income not to exceed $749 per month, $4,000 in total resources and $6,000 for couple. (Contact DHS for details)

SERVICES UNDER THIS PLAN

  • Personal Care Aide:
    Whatever your personal care needs are – from shaving and hair care to back rubs and bathing – our personal care services are uniquely tailored to meet your individual needs. (Care Plan developed by DHS nurse)
  • Homemaker Chore:
    This service may include shopping, running errands, personalized laundry and housekeeping. We can tidy up your apartment, wash your dishes, do your personal laundry and/or change your bed linens. (Care Plan developed by DHS nurse)

Home Healthcare OklahomaAdvantage Waiver Program

The Advantage Waiver Program is a Medicaid funded program that assists people with physical disabilities and who meet nursing facility level of care as stated under Medicaid, and also meet Medicaid financial guidelines as determined by the Department of Human Services.

PROGRAM ELIGIBILITY GUIDELINES

  • Must meet medical criteria.
  • Must meet medical eligibility for Nursing Facility level of care.
  • Income not to exceed $1692 per month, $2,000 in total resources and $6,000 for couple. (Contact DHS for details)
  • Individual must be able to remain safely in home during the time when services are not provided.

SERVICES UNDER THIS PLAN

  • Personal Care Aide:
    Whatever your personal care needs are – from shaving and hair care to back rubs and bathing – our personal care services are uniquely tailored to meet your individual needs. (Care Plan developed by Case Manager.)
  • Home Health Cherokee NationHomemaker Chore:
    This service may include shopping, running errands, personalized laundry and housekeeping. We can tidy up your apartment, wash your dishes, do your personal laundry and/or change your bed linens. (Care Plan developed by Case Manager.)
  • Medical Equipment/Supplies:
    Will cover the cost of certain assistive devices and medical supplies. (Need assessed by Case Manager.)
  • Advantage Case Management:
    Provides assistance for consumers from a certified case manager in gaining access to needed waiver and other state plan services, as well as needed medical, social, educational and other services, regardless of the funding source for the services to which access is gained.
  • Skilled Care:
    Includes medication set-up and monitoring by a nurse. (Care Plan developed by Case Manager)
  • Environmental Modification:
    To modify consumer’s property to ensure health and safety of the individual and to promote independence. Such adaptations may include the installation of a wheelchair ramp, grab bars or widening of doorways. (Need assessed by Case Manger)
  • Medication:
    Covers costs of some medications as needed.
